Resmetirom Mechanism of Action: How a Liver-Targeted THR-beta Agonist Treats MASH
Resmetirom Mechanism of Action: How a Liver-Targeted THR-beta Agonist Treats MASH
Last updated: October 2026. Scope: how resmetirom acts on thyroid hormone receptor-beta in the liver, why it largely spares thyroid hormone receptor-alpha, what that does to liver fat and blood lipids, how it showed up in the MAESTRO-NASH trial, and how it differs from semaglutide and saroglitazar. Based on the US prescribing information revised in July 2026 and the EU product information.
Resmetirom is a partial agonist of thyroid hormone receptor-beta (THR-β), the receptor form that dominates in the liver. Activating it increases fat burning in liver cells, which lowers liver fat, LDL cholesterol, apolipoprotein B and triglycerides. It acts much more weakly on THR-α, which carries thyroid hormone’s effects in the heart and bone. In the MAESTRO-NASH trial, this led to MASH resolution and fibrosis improvement at 12 months.
Resmetirom Mechanism of Action in Brief
Thyroid hormone helps set how fast the liver burns fat and clears cholesterol. It acts through two receptors: THR-α, which carries most of its effects on the heart and bone, and THR-β, the major form in the liver. Resmetirom is a synthetic small molecule that partially activates THR-β. In the liver, that increases fatty acid β-oxidation and reduces liver fat, including the lipotoxic fat that drives metabolic dysfunction-associated steatohepatitis (MASH).
Published reviews link MASH to reduced thyroid hormone activity inside the liver, sometimes called intrahepatic hypothyroidism, which may drive fat-related liver injury, based mainly on preclinical models. Resmetirom targets that signal in the liver while keeping off-target activity at THR-α, in tissues such as the heart and bone, to a minimum.
What Resmetirom Is Used For
Resmetirom, sold as Rezdiffra, is approved in the US and the EU for adults with noncirrhotic MASH and moderate to advanced liver fibrosis, stages F2 to F3, together with diet and exercise. MASH is the current name for non-alcoholic steatohepatitis (NASH), the inflamed and scarring form of fatty liver disease. The approval does not extend to simple fatty liver, mild fibrosis or cirrhosis. The drug is not yet approved in India; the current CDSCO position is tracked in is resmetirom available in India.
THR-β Versus THR-α: Why Selectivity Matters
The US label reports how resmetirom performs at each receptor in a functional assay:
| Measure | THR-β | THR-α |
|---|---|---|
| Where thyroid hormone acts through it | Mainly the liver | Heart and bone, among other tissues |
| Resmetirom response, as % of the T3 maximum | 83.8% | 48.6% |
| Concentration for half-maximal effect (EC50) | 0.21 µM | 3.74 µM |
Two things follow. Resmetirom needs about 18 times less drug to half-activate THR-β than THR-α, and the EU assessment describes it as 28-fold more selective for THR-β than the natural hormone T3. It is also a partial agonist: even at THR-β it does not reach the full response that T3 produces. The EU product information describes the result as minimal off-target activity on THR-α in tissues such as the heart and bone.
How Resmetirom Reaches the Liver
Resmetirom is taken once a day by mouth and reaches peak blood levels after about four hours. It is more than 99% bound to plasma proteins and has a half-life of 4.5 hours. Reviews note that it is carried into liver cells by OATP1B1, a transporter concentrated in the liver, and the label lists it as a substrate of OATP1B1 and OATP1B3. Food does not change its exposure in a clinically significant way.
The liver also clears it. CYP2C8 converts resmetirom to MGL-3623, a major metabolite that is 28 times less potent at THR-β. That pathway explains the label’s interaction rules:
- Strong CYP2C8 inhibitors such as gemfibrozil are not recommended with resmetirom.
- With moderate CYP2C8 inhibitors such as clopidogrel, the resmetirom dose is reduced.
- Resmetirom raises blood levels of some statins, so rosuvastatin and simvastatin are limited to 20 mg a day, and pravastatin and atorvastatin to 40 mg a day.
What Happens After THR-β Is Switched On
According to the EU product information, THR-β stimulation in the liver improves mitochondrial function and lipid metabolism and increases fatty acid β-oxidation, reducing liver fat, inflammation and fibrosis. The measurable effects are:
- Liver fat falls on MRI-PDFF and CAP scans. Liver stiffness and the ELF blood score also fall, though the US label says the clinical relevance of these changes is yet to be confirmed.
- Blood lipids fall: LDL cholesterol, apolipoprotein B, lipoprotein(a) and triglycerides. At week 52, LDL cholesterol was down 13.3% on 80 mg and 17.6% on 100 mg, against no change on placebo. Lipid reductions appeared from week 4.
- Free T4 falls by a mean 13% on 80 mg and 17% on 100 mg, against 2% on placebo, with minimal change in T3 or TSH. The US label reports no clinical findings associated with the FT4 decrease.
From Mechanism to Trial Results
The approval rests on the MAESTRO-NASH trial, in which adults with F2 or F3 fibrosis had liver biopsies before treatment and at month 12. Two pathologists read every biopsy independently.
| Month 12 endpoint | Placebo (n=294) | 80 mg (n=298) | 100 mg (n=296) |
|---|---|---|---|
| MASH resolution with no fibrosis worsening, pathologist A / B | 13% / 9% | 27% / 26% | 36% / 24% |
| Fibrosis improved by at least one stage with no MASH worsening, pathologist A / B | 15% / 13% | 23% / 23% | 28% / 24% |
Both doses reached statistical significance on both endpoints. ALT and AST also trended lower than placebo from month 3. Because biopsy findings are surrogate endpoints, the US approval is accelerated, and continued approval may depend on confirmatory trials that show clinical benefit.
What Resmetirom Does Not Do
- It is not thyroid hormone. It is a synthetic molecule that partially activates one thyroid hormone receptor, mainly in the liver, and it is not used to treat hypothyroidism.
- It is not for cirrhosis. The label advises avoiding it in decompensated cirrhosis and says its safety and effectiveness in MASH cirrhosis have not been established.
- It is not a weight-loss drug. The label makes no weight-loss claim.
- It has side effects. Diarrhoea and nausea were the most common. Pruritus, vomiting, constipation, abdominal pain and dizziness were also reported more often than on placebo, and the label warns about liver injury and gallbladder problems such as gallstones.
Resmetirom Compared With Semaglutide and Saroglitazar
| Drug | Target | MASH or NASH approval |
|---|---|---|
| Resmetirom | THR-β, partial agonist, liver-directed | US, March 2024 (accelerated); EU, August 2025 (conditional) |
| Semaglutide (Wegovy) | GLP-1 receptor agonist | US, August 2025 (accelerated) |
| Saroglitazar | Dual PPAR-α and PPAR-γ agonist | India (DCGI), March 2020, non-cirrhotic NASH |
Semaglutide acts on the GLP-1 receptor, and GLP-1 is a physiological regulator of appetite and calorie intake. Saroglitazar activates PPAR-α and PPAR-γ, receptors that govern lipid and glucose metabolism. Resmetirom is the only one of the three that works directly on the liver’s thyroid hormone pathway.
Why the Mechanism Matters for Resmetirom API Quality
None of this pharmacology reaches the liver unless the drug dissolves and is absorbed. Resmetirom has low aqueous solubility below pH 6 and higher solubility above pH 7 (0.44 mg/mL at pH 7.04), and the EU assessment records it as a claimed BCS class IV compound. Particle size and crystal form of the API therefore matter as much as purity. The drug substance specification assessed in the EU includes particle size by laser diffraction and polymorph identity by XRPD.
Bio-Synth manufactures Resmetirom API to a published specification of assay 98.0% to 102.0%, total impurities not more than 0.5% and any unspecified impurity not more than 0.1%, with a Certificate of Analysis on every batch.
The synthesis, impurity control and solid-form questions behind that specification are covered in resmetirom API manufacturing.
Frequently Asked Questions
How does resmetirom work?
Resmetirom partially activates thyroid hormone receptor-beta (THR-β), the main thyroid hormone receptor in the liver. This increases fatty acid oxidation in liver cells, which reduces liver fat, inflammation and fibrosis, and lowers LDL cholesterol and triglycerides in the blood.
What drug class is resmetirom?
Resmetirom is a thyroid hormone receptor-beta (THR-β) agonist, specifically a liver-directed partial agonist. Its ATC code is A05BA11, in the liver therapy group.
Is resmetirom a thyroid hormone?
No. Resmetirom is a synthetic small molecule that partially activates thyroid hormone receptor-beta, mainly in the liver. It does not replace thyroid hormone and is not used to treat hypothyroidism.
Does resmetirom affect thyroid function tests?
Yes, it lowers free T4 (FT4). According to the US label, FT4 fell by a mean 13% on 80 mg and 17% on 100 mg, against 2% on placebo, with minimal change in T3 or TSH and no associated clinical findings.
How long does resmetirom take to work?
Blood lipids fell from week 4 in clinical trials, and ALT and AST trended lower from month 3. The liver biopsy results that supported approval were measured at month 12.
Does resmetirom cause weight loss?
Resmetirom is not a weight-loss drug, and its US label makes no weight-loss claim. It acts on fat metabolism inside the liver rather than on appetite.
Can resmetirom reverse cirrhosis?
That has not been shown. Resmetirom is approved only for noncirrhotic MASH with F2 to F3 fibrosis, the label advises avoiding it in decompensated cirrhosis, and a trial in compensated cirrhosis is ongoing.
